The Dynamic Support Register (DSR) is a vital national tool designed to support the needs of people with a primary diagnosis of Learning Disability and/or Autism.
It helps identify those who may be at risk of crisis and ensures a coordinated response to meet their needs.
The referral criteria to the DSR is as follows:
- Those at risk of mental health hospitalisation/or being considered
- Places themselves or others at serious and/or significant risk of harm
- Where risk, mental health and/or lack of support is impinging upon community placement or tenancy, residential special school or care and support placement is at risk of irretrievable breakdown and where this would pose a significant risk to the safety and wellbeing of the person and/or others
- Is presenting to the police
- Is presenting to urgent and emergency care services due to mental health.
- Have had a recent discharge from a mental health hospital
- Have been supported by a mental health crisis team in the last year
- Have one or more factors that mean the person is at increased risk of admission (see DSR support guide link for further information on these factors )
This is not an exhaustive list and all of the above criteria may therefore require a multi-professional response, be monitored and/or prioritised for additional support in response to a person’s increasing needs or risks in which an enhanced support meeting such as the DSR is required to monitor their care and treatment as a team.
- Proactive Care: The DSR helps professionals respond quickly when a person’s needs change, preventing escalation into more serious issues.
- Crisis Prevention: By identifying those at risk early, the DSR helps reduce the likelihood of emergencies or mental health hospital admissions.
- Holistic Support: The register ensures that all aspects of a person’s life, school, home, education, and health are considered in their care plan.
- If you believe your child or an adult could benefit from being on the Dynamic Support Register, speak with your GP, school SENCO, or a healthcare professional. They will assess their needs and, if appropriate, refer them to be included on the register.
Self-referrals can be made to the DSR; however, this must be done with help from a health or social care representative.
Being part of the Dynamic Support Register aims to ensure that a person’s needs are closely monitored and that they receive the support they deserve. It's one more way we work together to create a safer, more supportive environment for people with complex needs.

You will be told:
- What information is being shared,
- Who your information will be shared with
- How your information will be used.
Your information will be recorded and shared for the purposes of the Dynamic Support Register, and this is done so to support safe and effective care.
Information discussed at the Dynamic Support Register is strictly confidential and will not be disclosed to third parties who have not signed up to the Dynamic Support Register Information Sharing Agreement without consent.
